Was it ever bundled with the standard or enterprise edition database?

    Don't know about V6, but I seem to recall that both Forms and PL/SQL
    were extra cost options in Oracle 7, at least early on. (7.0.x)

    AFAIK NO it was always an extra cost.

    Forms 1 or IAP was free as were Forms 2 and 3
