FAQ
I have upgraded from CM4.8 to CM5.0 and upgraded the agents as well.
However, when I restart CM and login, I get the
http://<server>:7180/cmf/upgrade-wizard/welcome page. I can confirm that
the agent is upgraded.

The wizard is shown unconditionally after a server upgrade, and it does
more than just upgrade your agents.

On the first page, it asks if you want to upgrade the agents or skip this
step. In your case, you should skip it, and let it check if there's any
other work it needs to do. if there isn't any, it will simply finish, and
    you won't see it again.

    Philip,

I have already upgraded the agents and I do not want to see the wizard
again (unless I manually run it through hosts page). Is there a way to do
    it?

As Phil said, go through the wizard and click skip for the agent upgrade
step. When you complete the wizard, it will not show up anymore.

Until you complete the wizard, it will keep popping up.

I completed the wizard at least half a dozen times but still I see the
    wizard pop up again.

You got to the end, clicked 'Finish' and it keeps coming back every time
    you log in?
